To register for the Holter Monitor, please complete the form provided on this page. Ensure that all information you enter is accurate, as it will be used to arrange your monitoring payment link. In addition, please upload a referral letter from your GP or doctor in PDF or Word format. This referral is required for us to proceed with your registration and will allow us to coordinate with your healthcare provider to ensure the best care. Once your registration is received and reviewed, you will be contacted with further instructions.
